CTBS Recertification Extension Request Form

If you are unable to complete your continuing education requirements before your CTBS certification expires, you may request an extension by completing this form. Temporary waivers may be granted for active military duty, medical hardships, or natural disasters. Requests must be submitted before the certification lapse date and include supporting documentation.
